Public Integrity Division Complaint Report

The Public Integrity Division investigates the following types of complaints and election law violations:

  • Voter fraud, including absentee ballot fraud

  • Voter registration fraud, such as multiple registrations, registrations in more than one state, or destroying or not submitting voter registrations

  • Voter intimidation

  • Voter interference, such as scam mailings or scam calls

  • Vote buying

  • Violations of Ohio elections laws, such as wrongdoing by election officials, harm or threat of harm to the conduct or administration of an election, or falsification of election records

  • Violations of Ohio campaign finance laws, such as falsification of records

This complaint form should be used to report these types of violations.

Ohio law requires specific information to be included in the complaint for the Ohio Secretary of State’s office to begin an investigation.

All complaints must contain the following:

  1. Personal Information

    1. Name and address of all parties, including the individual making the complaint and the individual about whom the complaint is being made

    2. If any party is a campaign committee, the name, title, and address for both the candidate and campaign treasurer

    3. If any party is an attorney, the name, title, physical address, email address, and phone number

  2. Statement of Facts

    1. A written narrative or statement of the alleged violation(s) with an explanation of how the complainant has personal knowledge of the complaint

  3. Reference to Applicable Ohio Law

    1. Each alleged violation of the law must be stated and cross-referenced with the relevant section of the Ohio Revised Code

  4. Supporting Material

    1. Affidavits, exhibits, or other documents must be attached to the complaint

  5. Signed under penalty of perjury

    1. I certify that the information contained in this complaint is true and correct to the best of my knowledge, information, and belief. This complaint is submitted pursuant to R.C. 3517.16, and I understand that knowingly providing false information may subject me to penalties under Ohio law.
